RESUMO

INTRODUCTION AND IMPORTANCE: There are only a few case reports to date that have described patients with three or more multiple primary tumors. However, they have been reported more in the last decade, so a precise screening in patients with or without risk factors could be helpful in early diagnosis and treatment. This work has been reported in line with the SCARE criteria. CASE PRESENTATION: Here, we presented a 44-year-old female patient without any history of smoking, alcohol consumption, or cancer in her family. She had three metachronous primary tumors; breast, thyroid, and gastric cancer, which had metastasized to both her ovaries and colon. She died in January 2023 due to complex pneumonia and septic shock. To our knowledge, this article is the second case in which breast, thyroid, and stomach cancer are reported together. CLINICAL DISCUSSION: When it comes to Multiple primary malignancies (MPMs), not only screening in patients with risk factors should be considered, but patients without any other risk factors except current or past history of tumors should be screened precisely for early diagnosis and treatment. In this study, we discuss prevalence and causes of MPMs, prevalence of breast, thyroid, and stomach cancer, and also their possible relations with each other that may affect their occurrence. CONCLUSION: Reporting other cases with MPMs by physicians could lead to establish an evidence based approach to these patients.

RESUMO

Key Clinical Message: Peritoneal encapsulation is most of the time asymptomatic and is found incidentally, but when symptomatic it usually presents with bowel obstruction. CT scan is a gold standard for the diagnosis of bowel encapsulation. Abstract: Congenital peritoneal encapsulation (CPE), abdominal cocoon syndrome (ACS), and sclerosing encapsulating peritonitis (SEP) are syndromes in which the small bowel is encapsulated. Small bowel encapsulation is usually asymptomatic and rarely presents with small bowel obstruction. In this article, we report a 65-year-old man who presented to our hospital with signs and symptoms of small bowel obstruction. He underwent an urgent operation, and small bowel encapsulation was diagnosed. One year after the surgery, all symptoms improved.

RESUMO

The main barriers to cells or organ transplantation such as pancreatic ß-cells are the need for lifelong immune suppression and the shortage of donors. It may be overcome via cell encapsulation and transplantation techniques. Hydrogels provide a suitable ECM-like microenvironment for cells to adhere, survive, and function, while weakly performing as an immune barrier. In this study, we aimed to macro-encapsulate islet cells in a dual encapsulation device with collagen hydrogel and PCL nanofiber to provide an immune-isolated environment for cells to function more efficiently, where immune cells are not allowed to enter but oxygen, insulin, and nutrients can pass through. PCL thin mats with the pores diameter of 500 nm were synthesized by electrospinning and characterized by scanning electron microscope, porosity measurement, tensile strength test, and contact angle measurement. Collagen hydrogel was fabricated by extracting collagen fibers from rat tail tendons and solving them in acetic acid. ß-cells (CRI-D2 cell line) encapsulated after neutralizing collagen solution (pH ≈ 7.4). Cell-collagen gel complex was poured into the nanofibrous mat packets to fabricate the whole device. Histology evaluation, cell viability, and cell function tests were done in 10 days. Live/dead assay of Cri-D2 cells encapsulated within the device showed that cells have diffuse distribution at the core of the hydrogel and the device. Also, cluster formation was seen and shows these cells can live in groups. To identify cells' function within the device in these 10 days samples' supernatant insulin level was measured by chemiluminescent immunoassay. It just showed a positive result for existing insulin within the medium. Based on our results, this device presents adequate features to be a good immune-isolation device for cell transplanting.

RESUMO

Idiopathic granulomatous mastitis (IGM) is a rare benign infectious disease of the breast, commonly presenting with a unilateral breast mass. Since GM's clinical presentation and imaging can be very similar to breast cancer, diagnosing GM can be challenging. So far, various reports have demonstrated the probable correlation and co-occurrence of granulomatous mastitis and breast cancer. This report presents a 38-year-old female with invasive ductal carcinoma, previously diagnosed as IGM.
